New Patient Registration

Anyone who lives within our practice area, the details of which are below, can register with the practice by attending surgery.

Following registration form acceptance on the day (please bring list of medication with you when you request to register) you can book an appointment to see a GP at that time.  You will then be asked to book an appointment for a new patient health check with one of our Practice Nurses. Photographic identification and proof of address is required for registration. Please ask at reception for what is required if you are unsure.

Medical treatment is available from the date of registration. Please contact reception for further information.

We are also accepting patients who live outside our area who wish to be seen in practice but cannot offer home visits for patients outside our area.  All other services are available to those patients apart from home visiting.  Please ask staff for details.

 

 

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. Please ensure both of the forms below are completed.

We are also now including all L16, L18, L25, L26 and L27 post codes in our practice boundary
